When a diagnosis is repeatedly downgraded, is the query always the problem?
Not necessarily. And the payer isn’t automatically right, either.
Three questions often get conflated:
Clinical validity: Is the diagnosis supported by the clinical evidence?
Coding compliance: Was it coded in accordance with applicable rules?
Reimbursement: Does it meet the payer’s contractual requirements?
A clinically valid diagnosis does not inherently require increased resource utilization or a dramatic change in clinical trajectory. The clinical criteria for the diagnosis—not an assumed level of resource consumption—must drive the assessment.
Before blaming the query or accepting the denial, establish which question is actually in dispute.
